I guess people did try to argue that even when the film was released, but Bill was not really Transgender anymore than someone like Norman Bates.

According to the script, Bill had applied for gender reassignment but was denied because he failed the psych eval. He was transgender but denied physical conversion. So he killed women to use their skin to make a woman suit for himself. his metamorphosis like the moths.

Well, I interpreted failing the evaluation as showing signs he wasn't necessarily transgender. Before this he was just homosexual with Raspail who he caught cheating with Klaus and skinned him.

Billy hates his own identity, you see, and he thinks that makes him a trans-sexual. But his pathology is a thousand times more savage and more terrifying.


I understand transgenderism as a dissociative disorder. You don't identify as what you physically are. So, they are implying he has a dissociative disorder, therefore he couldn't be transgender. To me that scripting doesn't make sense. It is the belief that you are something other than what you are that makes a person transgender. Otherwise, they'd just be a gender.
 
I could imagine his psych session:


Grumb: I feel like I'm really a woman.
Lector: No you don't. You just hate yourself.

That is what I'd tell a crazy person if I wanted them to go off the handle.
