The NYT kept calling him Mr, so I'm pointing out all his sir names, but how Mr. is not one.

Captain, yes.

I think he probably would rather spend time talking about his message than himself during those all important debates. I happen to appreciate it, when most of what the others do is puff their chests up and try to tell us they're superman.

Mr. is an appropriate way of addressing officers in general. Military doctors are just about always officers. e.g. 2nd Lieutenant and up or Ensign and up

Captain in the Air Force would be like Lieutenant in the maritime services. 2nd Lieutenant in the land services is like Ensign in the maritime services......1st Lieutenant ------Lieutenant Junior Grade in the maritime services.

I believe doctors start out as captains. At least that is how it was when I was in the Air Force in the 90s.

I never did see a doc above major (1 rank above captain). I assume that getting rank as a doc is hard or docs in the military dont stick around.

Lt Cols (0-5)are often choose for the leadership of medical units. You have 12 years as captain today to move up or move out of the military. I am sure it was different back in the day.

You are right. I believe that they come out of OCS as a 1st Lt. and 6 months after attain captain. Only drawback is that promotion comes VERY slowly afterwards.

Technical skills often get minimum ranks and plane pilots and doctors are officers in the US system. Interestingly, the MASH TV show paid attention to show nurses as baseline lieutenants and doctors as baseline captains.

RP was at least a captain but I have not been able to confirm if that was his final rank after Air National Guard discharge.
