Considering how old he is... and the work his wife likely gets on her face - I'm sure he has spent a bit of time getting his face worked. Thats probably part of it. Theres an unnatural fullness to the guys face for a 70+ year old man.

He fought at least couple of bouts of melanoma. He is likely on some immune treatment or low intensity chemo. That may account for his face getting deformed and puffy as well as the apparent mental sluggishness.
