I've been reading the opinion bit by bit (not the text of the original act however) and I believe this is correct. If the state does not set up the exchange the feds do it for them and run it themselves. Frankly, I don't know which would be worse.

My 17 year old stepson made this decision for himself. I admire him more and more everyday for accepting the inevitable and choosing to die at home with the love of family and friends surrounding him rather than attempting to buy a few more months of life by continuing chemotherapy.
